Investigation launched after Glasgow hospital releases wrong body, leading to mistaken cremation

An investigation has been launched after a Glasgow hospital mistakenly released the wrong body to undertakers, leading to a cremation being carried out in error.

The mistake at Queen Elizabeth University Hospital was only discovered after the funeral service and cremation had already taken place. As a result, another family was unable to have their relative’s remains present at their own funeral.

NHS Greater Glasgow and Clyde, which runs the hospital, said the incident was caused by human error after staff failed to follow procedures for identifying and labelling bodies before their transfer from the mortuary. The health board has apologised to both families affected.

The incident, which reportedly occurred last month, is now the subject of a full investigation. Staff involved have been suspended while inquiries continue.

Dr Scott Davidson, medical director at NHS Greater Glasgow and Clyde, offered “sincere apologies” and said it was deeply regrettable that established safeguards were not followed, causing additional distress to families at an already painful time.

The hospital, the largest in Scotland, is already under a public inquiry following previous infection outbreaks and concerns over water and ventilation systems. In September, former board chair Jane Grant was questioned about the hospital’s communication with patients and families.
