Matthew Perry's death certificate reveals new details surrounding the beloved actor's death.
The Friends star passed away at his LA home on October 28.
His death certificate, obtained by The Blast, details the official details of his passing. It lists the informant as Matthew's step-father, notable Dateline journalist Keith Morrison. The document also confirms the authorities noted his time of death at 4:17 p.m. PST.
While unconfirmed, sources believe Matthew's live-in assistant of 25 years, Kenny Iwamasa found the actor's body and made the call to 911. The death certificate did not confirm that this was the case, but did reveal where Matthew was buried during his Nov. 3 funeral.
According to the death certificate, he was buried at Forest Lawn in Los Angeles. His family and friends, including his Friends co-stars, came together last week for the actor's funeral at the cementory.

Despite all these details, Matthew's official cause of death is still yet to be determined. Although emergency responders walked into a water rescue situation and he was found dead on the scene, the details are still being evaluated and investigated by authorities.
Initially, authorities reported there were no drugs on the scene. A recent toxicology report confirmed that Matthew did not die from a methamphetamine or fentanyl overdose. Dr. Michael Baden, former chief medical examiner of New York City, told Fox News Digital: “They’ve got pretty much 99% of what they’re going to get at this point as far as information goes, and they can release it now, or they can wait for a couple more weeks because they’re looking for some unknown drug. They might be doing microscopic slides, but it’s now about almost two weeks, so the toxicology should be finished.”
However, his cause of death was listed as "deferred" for now, meaning the investigation is still pending and needs more time to be completed. Chief of Coroner's Investigations Craig Harvey told Inside Edition that they are still investigating and doing tests to determine if prescription drugs were involved. Moreover, no foul play was suspected at the time.
After the news of his death, the Friends cast made a joint statement. They said: "We are all so utterly devastated by the loss of Matthew. We were more than just castmates. We are a family. There is so much to say, but right now we’re going to take a moment to grieve and process this unfathomable loss.”
